Contact details and birth date
Referrer contact details
Reason for referral - include teeth numbers
Relevant patient clinical images, ie, photos and/or radiographs - OPG preferred
List all medications or supplements the patient is taking - especially blood thinners or anti-resorptives
Is antibiotic prophylaxis needed?
Any past head and neck radiotherapy?
Any past hospitalisations?
Any cognitive, behavioural, or mobility issues?
How dentally anxious is the patient - nil, mild, moderate, or severe?
Consent from the patient to share health data
